Please note that some documents have 4 versions to reflect the delivery and expectations of different teams (Adult Tailored Learning; Apprenticeship; Young People's Learning; Functional Skills).
If you are delivering on Multiply courses then please continue to use last years documents which can be found Course: Multiply - Quality Documents (participationandlifelonglearning.co.uk).
We have made 2 versions of the HA16c using MS forms, one for the start of the course and one for the end of course that you can use with online courses.
The links below allow you to duplicate the forms, which you can then rename for each specific course. If these are been generated centrally then to enable the tutor to see the results you will need to add them as a collaborator (under the share tab)
The link below allows you to duplicate an MS Form to create an electronic version of HA19 (end of course evaluation). The Form will allow you to create an Excel spreadsheet of the responses which will help you analyse the feedback
